Medical information and consent

To provide safe and elite-level coaching and learning environments, we need all participants to complete this registration form. We collect this data to:

  • ensure our coaches are aware of any medical or neurodivergent needs to support players and athletes effectively
  • manage emergency contact details.
  • secure the necessary consents for technical video analysis and reporting

I understand that the player or athlete must wear appropriate clothing or kit to participate in the sessions (our coaches will advise on required kit)*
I understand that MotionLab maintains high coaching standards and a positive environment. If a player or athlete's behavior is deemed unacceptable, they may be asked to leave*
I acknowledge that if a player or athlete is unable to participate due to a lack of appropriate kit or unacceptable behavior, no refund will be provided*
I consent to photos and/or video of the player or athlete to be used by MotionLab for promotional purposes (including on social media)*
I consent to the player or athlete being recorded for coaching and technical analysis purposes*

I understand my data will be stored and processed for safeguarding, coaching, and planning purposes. I am aware that I can ask for my data to be deleted at any time.*
